ANSWER

Your employer can offer rewards or penalties to encourage employees to take a health risk assessment. As long as your employer doesn't require you to meet a health standard that the assessment will reveal, such as a target body mass index (BMI) or cholesterol level, then your employer is not limited in the rewards or penalties imposed for participation in a health risk assessment.
